RISING 6TH GRADE STUDENTS

The Virginia General Assembly requires all 6th grade students to have a tetanus, diphtheria, pertussis (Tdap) booster shot prior to entry into the school this September, if at least 5 years has passed since the child's last shot. Please review your child's immunization records.  Contact your doctor or local health department if you have any questions.  Documentation of the Tdap should be taken to your child's school nurse.

REASONS TO STAY HOME FROM SCHOOL
  • Nurse's judgement or parental discretion
  • Fever over 100 orally within 24 hours
  • Active vomiting within 24 hours
  • Diarrhea within 24 hours
  • Swollen or Painful joints from injury
  • Severe Earache
  • Suspected contagious disease (until on treatment for 24 - 48 hours)
  • Chickenpox
  • Conjuctivitis (Pink Eye) (until on treatment for 24-48 hours)
  • Scabies
  • Undiagnosed Rash
  • Injury or Illness requiring further immediate treatment
  • Suspected Substance Abuse
  • Preschoolers- continual yellow or green drainage from nose

Hearing and Vision Screening
Hearing and Vision Screenings are performed on the following students at Hampton Oaks Elementary:
  • All Kindergartners
  • All 3rd Graders
  • All new students to Stafford County (regardless of grade-level)
  • All teacher/parent referrals
  • Special education students as determined by the Child Study Team
If a student is suspected to have a deficit after a screening, a referral letter is sent home recommending a follow-up visit with the appropriate specialist (i.e. audiologist for hearing, and ophthalmologist/optometrist for vision). *If no problem is noted, no notice is sent home.*
 
Please make sure that children who have been prescribed glasses bring them to school every day so that they may be prepared to do their best.

Also, if your child needs glasses and you are not able to afford them, you may qualify for assistance through LIONS Club. Applications are available from the Clinic. Please call or send a note, and I will be happy to send an application home with your student.
